Registering with the practice

Registration can be done by email or telephone or online when given access by us.  In order to register with our practice by email or telephone  we will need your name, date of birth, address, phone number and email address.  By registering with our practice you agree and give us your consent for us to store this information about you in our clinical system in preparation for your first consultation.
